INCIDENT NARRATIVE
During a private evaluation of prototype PROT-006 (Spatial Feedback Loop Array) in Studio A, an unintended positive feedback loop formed between the central 16-channel microphone tree and perimeter horn drivers. The algorithm's automatic gain limiter experienced a 120ms latency delay, allowing an 840 Hz room resonance mode to escalate into continuous acoustic saturation at 116 dB SPL. The test session was immediately aborted via master emergency stop.
ROOT CAUSE ANALYSIS
A race condition between the Max/MSP DSP scheduler and the Dante network audio driver delayed gain reduction packets by 6 buffer cycles, disabling software limiting during transient peaks.
CONTAINMENT PROTOCOL
Hardware analog optocoupler limiters were permanently spliced directly into power amplifier input stages, bypassing software control.
