INCIDENT NARRATIVE
During high-amplitude resonant testing of a 1.2-meter sintered basalt tone slab (PROT-002), high-output piezoelectric actuators swept through a continuous 800–900 Hz frequency range. At 842 Hz, internal mechanical resonance concentrated stress along a microscopic olivine vein, causing the stone slab to fracture cleanly into two unequal segments.
ROOT CAUSE ANALYSIS
Natural geological inclusions in raw basalt concentrate mechanical strain under continuous acoustic excitation. Resonance drive lacked dynamic amplitude throttling.
CONTAINMENT PROTOCOL
Ultrasound non-destructive testing now required for all mineral specimens prior to cutting. Maximum drive amplitude capped at 65% of mechanical shear limits.
