INCIDENT NARRATIVE
During calibration of a high-power somatosensory tactile floor array (PROT-023) designed for whole-body listening, continuous 14 Hz sine waves were injected into the timber sub-floor. Unknown to technicians, 14.2 Hz matched the mechanical resonant frequency of the building's suspended ceiling grid, causing ceiling tiles to rattle loose and fall.
ROOT CAUSE ANALYSIS
Failure to isolate the acoustic test platform from building architectural structure using elastomeric vibration dampening pads.
CONTAINMENT PROTOCOL
Mounted all tactile transducers onto decoupled dual-layer Sylomer acoustic isolation mats. Infrasonic sine sweeps capped at 95 dB.
