INCIDENT NARRATIVE
During listening evaluations of a new 5th-order ambisonic decoding matrix in Studio A, an engineer inadvertently swapped polarity (+/-) on four overhead height speakers. Subjects reported confusing auditory sensations, head pressure, and difficulty pin-pointing sound source trajectories.
ROOT CAUSE ANALYSIS
Manual wire termination error during speaker patching combined with lack of an automated pink-noise phase validation script.
CONTAINMENT PROTOCOL
Developed an automated phase and delay calibration script that tests every loudspeaker node before each evaluation session.
